  • Weekly 4CW face to face coaching (30 min/wk)
  • Weekly assessment regarding upcoming week’s plan care and responsive education opportunities
  • Individualized coaching regarding existing healthcare/oncology resources at their place of cancer care and within the community - initially and as needed (60 min initially & then included in coaching)
  • Individualized weekly exercise program including interactions with exercise trainer from "Maple Tree Cancer Alliance"
  • 4CW coaches collaboration with Oncology Nurse Navigator at their cancer center at least monthly with permission from the client. (30 min/monthly)
